Change of Address

This form is to be used to change your address or telephone number. The completed form, including the Policyowner’s signature, should be returned to our office at the address provided on the form.

Change of Beneficiary

This form is to be used to change the Primary Beneficiary designation on your policy. It is also to be used to change or add a Contingent Beneficiary. It is important to review the “Points to Consider” regarding beneficiary designations on Page 2 before completing the form. The completed form is to be returned to our office at the address provided on the form.

Change of Name

This form is to be used to change the name of the Life Insured/Annuitant, Policyowner, Contingent Policyowner, Beneficiary or Contingent Beneficiary. If the name has been changed due to a legal name change or legal adoption, supporting evidence must also be provided. The completed form, along with the supporting evidence, if required, is to be returned to our office at the address provided on the form.

Change of Banking Information

This form is to be used to change the bank account from which the Monthly Automatic Cheque (MAC) premium withdrawals are being made for your policy (ies). The completed form, along with a “VOID” cheque, should be returned to our office at the address provided on the form.

Please note that we require at least 10 days prior to the withdrawal date to make a change. Due to the nature of the MAC system, the banks require lead time to process the MAC withdrawals. Unfortunately, a MAC withdrawal cannot be stopped once it has entered the banking system.

PDF Printing Problems

Some printers do not have enough memory in order to print PDF files from the internet and you may not be able to print one or more of Wawanesa Life forms. This problem is easily fixed by adjusting your Adobe Reader settings. To change your settings:

  • Start the Adobe Reader program by double-clicking on your Adobe Reader icon.
  • Click on the Edit menu and choose Preferences.
  • Click on the Internet tab.
  • Remove the checkmark from the “Display PDF in browser” preference and click OK.

Note: You must restart your computer for this change to take effect.
